Found this really cool tutorial which has a simplistic way of presenting this web forms (checkboxes, radio buttons, etc).

Unfortunately, they do not go further to illustrate how the user input data can be collected, but I found their explanation on how to create the web forms a bit easier to follow. check on page 78.

PS: They also explain the extra feature of creating links to different parts of the same web page, Eg, from the bottom, creating a link that takes user back to the top, or links to take user directly to a particular paragraph on the same web page.

Done! I learnt how to do this in IT class in high school, but completely forgot what it was called. Why do they call it "radio" buttons and not something simpler like "choosy" buttons spinning smiley sticking its tongue out
