How do I got about adding a file, eg. Out.txt to a qt4 project?

I thank you must click the link "File" then you select "new file or project", under "files and classes" you select C++ or you can select Generals... then you can I think you can take it from there.

Thanked I've done this but i keep getting an Idreturn exit status, I googled this and it seems to suggest that it can't find my text file.
Any suggestions?
