Booking for EUP practicals and theory

I'm just trying to find the right document that explains how to book for the EUP 2 practical and one theory exam.
From what I know, one exam booking deadline is 31 May 2010 so if you guys know what is the process please
post it here or the relevant tutorial letter or even an online link I'd appreciate it.
